SAFETY
INCIDENT STATISTICS
An organisation's safety performance is dependent on the robustness of incident investigation, classification and reporting.
 
All incidents, including near misses, must be reported & investigated
 
Line management must be trained in incident investigation and must lead all investigations. Safety professionals to facilitate, support and advise
 
The purpose of investigation is to unearth and fix the root (systemic) cause and not blaming anyone

Incident Reporting
Project Parivartan has led to increased transparency in reporting of incidents. We have recorded 5,000 incidence reports in FY15 compared to just 731 in FY12. 90% of these incidents are low-potential near miss incidents.
 
Near Misses
In FY14, 54% of reported incidents were Near Miss Incidents (NMI), whereas in FY15, 89% of reported incidents were NMI. Near miss1 reporting has shown improvement in Formulations units, API units and Biologics, while IDPO and CPS have scope for improvement.
1 - A 'near miss' is any incident or emergency which did not result in an injury.
